You will need to complete the application for Professional Edge Nursing. You will also need to fill out our payroll company information, LEVER 1.
TB test
In the application it will ask you to provide information on your TB testing. If you have ever tested positive, you will be required to include results from latest chest x-ray and any medication you are taking because of it.
Identifications
You will need to provide copies of your identification. You will also need to have your I-9 notarized by a Notary Public. You also must turn in a copy of your RN license. If your state does not issue licenses, then a print out from the nursing website will do.
resume
If we do not have an updated copy of your resume, please include this when submitting your application.
insurance
You will need to provide a certificate stating that you have the required amount of Professional Liablity Insurance.
You will not need this until you are hired.
patient responsiblity
You will also be require to sign off on a form stating that you understand that you are providing a service for the patient and the client. You are responsible for putting in a required minimum 2 week notice and that you may not just stop seeing a patient. You are also required to complete all documentation on a patient even after you have resigned. Abandoment issues will be dealt with swiftly and promptly.
